All Pre-Kindergarten, Kindergarten and 7^th grade students will be asked to update their immunization records, as these are primary years of booster vaccinations.

If you choose to not immunize your child, you must file for an exemption through the State of Oklahoma. Please note a completed and signed exemption form is required prior to enrollment according to Oklahoma Statute 70 O.S. § 1210.191   Additional information as well as access to the online application can be found at https://oklahoma.gov/content/dam/ok/en/health/health2/aem-documents/prevention-and-preparedness/immunizations/IMMY_Exemptions_ENG.pdf  Forms must be completed and submitted to the school by the students parents. Students without the proper immunizations or completed exemptions will not be allowed to start school. If filling out an exemption, please read all rules and guidelines prior to submitting.
